Quick Reference

Appointments require at least 24 hours notice to cancel. Late cancellations and no-shows may incur a fee of up to $75. Completed services are non-refundable. Programs involving pre-appointment lab work are subject to a minimum retained fee of $200 once a lab order has been issued. Unopened physical products in original factory seal may be returned subject to the conditions outlined below.


Overview

These Practice Policies govern the clinical relationship between Fresh Fit Consulting, operated by Iza Boesler MD, and patients and prospective patients of the practice. By scheduling an appointment, submitting a patient intake form, or making a payment, you acknowledge that you have read and understood these policies.

These policies apply to all services offered by Fresh Fit Consulting including internal medicine, sleep medicine, and restorative medicine services.


Physician-Patient Relationship

A physician-patient relationship with Fresh Fit Consulting is established upon Dr. Boesler's acceptance of a patient and the commencement of clinical care at the first in-person appointment.

Visiting this website, submitting a contact form, completing a patient intake or screening questionnaire, or making a payment does not establish a physician-patient relationship. Dr. Boesler reserves the right to decline to enter into a physician-patient relationship with any prospective patient following review of their intake information, without obligation to provide a reason.

Once established, the physician-patient relationship continues until formally terminated by either party in accordance with applicable New Jersey law and medical ethics guidelines.


Active Patient Status

To receive courtesy patient services such as medication refills, same-day sick visits, or appointment referrals, patients must maintain active status by completing a scheduled appointment within the preceding six months.

Maintaining active patient status ensures that Dr. Boesler has a current and comprehensive understanding of your health before providing ongoing guidance. Regular visits allow for monitoring of health changes, adjustment of treatments as necessary, and continuity of care. Without a recent evaluation, it is not possible to offer appropriate clinical guidance or manage ongoing health concerns responsibly.


Visit Types and Scope

Fresh Fit Consulting specializes in internal medicine, sleep medicine, and restorative medicine. Each area of focus requires dedicated clinical attention. To optimize your care experience, patients are asked to clearly specify the desired discipline of care before each appointment.

Internal medicine covers the diagnosis and treatment of general health concerns. Sleep medicine covers the evaluation and treatment of sleep disorders including insomnia, sleep apnea, and circadian rhythm disturbances. Restorative medicine covers personalized hormonal, metabolic, and preventative care using functional testing and physician-guided treatment protocols.

Patients with concerns spanning multiple disciplines will need to schedule separate appointments to ensure adequate time and attention is given to each area. This approach reduces wait times and ensures that each patient receives focused, thorough, and efficient care.


Review of Testing Results

All patient test results ordered by Fresh Fit Consulting will be reviewed in a face-to-face meeting with Dr. Boesler. This ensures that results are thoroughly explained, questions are answered, and appropriate next steps are discussed in the context of your full clinical picture.

For normal results that do not require clinical follow-up, patients will be verbally notified. Patients who wish to discuss normal results in further detail are welcome to schedule a follow-up appointment at their convenience.


Insurance and Financial Responsibility

Patients are responsible for understanding their insurance provider's pre-authorization requirements and coverage policies before receiving services, including laboratory testing. Fresh Fit Consulting cannot guarantee pre-authorization approval or insurance coverage for any services or tests rendered. Financial responsibility remains with the patient in cases of denied or partial coverage.

Fresh Fit Consulting is not responsible for negotiating service fees with third party insurance providers. In the event of a coding error or illegible order, revised ICD codes will be provided directly to the patient or via fax to the requesting third party. For coverage details, patients should contact their insurance provider directly.

Fresh Fit Consulting provides courtesy insurance claim submission for patients with Original Medicare, Medicare Advantage, and Veterans insurance plans receiving services within the scope of insurance guidelines. Patients with private insurance are provided all necessary documentation including itemized receipts with CPT and ICD-10 codes to self-submit claims after payment for eligible services.


Restorative Medicine Disclosures

In addition to traditional internal medicine and sleep medicine services, Fresh Fit Consulting offers an expanded group of services under the category of restorative medicine. These services go beyond those traditionally available under insurance coverage and provide insight into metabolic function, hormone management, functional testing, and medication alternatives.

All Fresh Fit Consulting patients, regardless of participation in restorative medicine services, are asked to review the following:

Voluntary participation. Participation in restorative medicine services is completely voluntary. Patients may choose not to engage in these services and continue to receive care at Fresh Fit Consulting within the scope of conventional internal medicine and sleep medicine.

Insurance coverage and fees. Payment for restorative medicine services is due at the time of service. Restorative medicine services are not eligible for insurance claim submission of any kind.

Effectiveness and treatment strategy. Restorative medicine treatments are supported by clinical research and physician experience but may extend beyond the standard recommendations of the American Board of Internal Medicine. Restorative treatments complement conventional medical care and do not replace standard medical treatments. They frequently serve as adjunct therapies and are applied alongside conventional care where appropriate.

Dietary protocols and supplements. Fresh Fit Consulting may recommend specific dietary protocols, nutritional supplements, and herbal supplements to support metabolic and biochemical function. These recommendations are made for therapeutic purposes and are intended to support overall health. Nutritional supplements can generally be used alongside pharmaceutical medications, but interactions may occur. Patients are strongly encouraged to inform all of their healthcare providers about any medications, supplements, or hormones they are taking. Fresh Fit Consulting cannot predict or guarantee the absence of interactions in all cases.

Supplement purchases. Fresh Fit Consulting offers high-quality nutritional supplements sourced from reputable manufacturers who conduct rigorous third-party testing. Purchase of supplements from Fresh Fit Consulting is entirely optional. Treatment plans are never dependent on purchasing supplements from our office. Fresh Fit Consulting cannot guarantee the quality or equivalency of supplements obtained from outside sources.


Phone, Text, and Email Policy

Fresh Fit Consulting prioritizes serving patients with scheduled appointments. While we strive to respond to all calls, text messages, and emails promptly, our primary focus remains on providing quality in-person care.

If we are unable to answer your call immediately, please leave a detailed message including your name, date of birth, reason for calling, and the best contact number. Messages are reviewed and returned based on clinical urgency. Non-urgent inquiries may also be directed to info@ffcmed.com.

For medical emergencies, patients should call 911 or visit the nearest emergency room immediately. Fresh Fit Consulting does not provide emergency medical services via phone, text, or email.


Telehealth Visits

Restorative consultations are available in-person or virtually. Virtual visits are conducted through a secure platform and are subject to the same scope, policies, and fees as in-person consultations. Patients scheduling a virtual visit are responsible for ensuring they have a stable internet connection and a private environment for the duration of the appointment.


Cancellation Policy

Fresh Fit Consulting understands that unforeseen circumstances arise. If you are unable to keep a scheduled appointment, please notify our office at least 24 hours in advance. This allows us to offer the appointment time to another patient in need of care.

Patients who do not provide at least 24 hours notice of cancellation or rescheduling may be subject to a late cancellation fee of up to $75. Patients who fail to attend a scheduled appointment without notifying the office will incur the same fee. Late cancellation and no-show fees are not covered by insurance and are the sole financial responsibility of the patient.

Exceptions may be made at the sole discretion of Dr. Boesler in cases of documented emergency or extenuating circumstance.


Refund Policy

Fresh Fit Consulting strives to provide exceptional care and clear expectations around financial commitment. Please review the following refund terms before scheduling or submitting payment.

Completed services. Completed services and consultations are non-refundable. Once a service has been rendered, the associated fee is earned and will not be returned regardless of outcome or patient satisfaction with results.

Programs involving pre-appointment testing or laboratory orders. For programs that include a pre-appointment laboratory order or preparatory clinical review, program fees are allocated across defined service milestones. Once a milestone has been completed, the corresponding portion of the fee is non-refundable.

Cancellation requested before a lab order has been issued or preparatory clinical review has begun may be eligible for a full refund at Dr. Boesler's discretion, less any applicable credit card processing fees.

Cancellation requested after a lab order has been issued or a clinical review has been completed will result in a minimum retained fee of $200 to cover physician time for questionnaire review and lab order preparation. The remaining balance may be refunded at Dr. Boesler's discretion.

Multi-session and protocol programs. For programs consisting of multiple sessions, consultations, or sub-services, refund eligibility is determined by subtracting the cost of all rendered sub-services from the total program fee. A minimum of $200 is retained regardless of the number of sub-services rendered. The remaining balance, if any, may be considered for a partial refund on a case by case basis. Partial refunds under this provision are not guaranteed and are evaluated at Dr. Boesler's discretion.

Patients seeking a partial refund for an incomplete program are encouraged to contact FFC directly to discuss their specific situation.

Physical product returns. Physical products may be returned if they are in their original factory seal and remain unopened, and carry an expiration date of at least 90 days from the date of return. Products expiring in less than 90 days from the date of return are subject to a 25 percent restocking fee unless purchased within the last 10 business days. Opened or used products are not eligible for return.

Credit card transaction fees. Credit card transaction fees are non-refundable in all circumstances. These fees are retained by the payment processor and cannot be returned by Fresh Fit Consulting.

Refund method and processing time. Refunds for payments made via cash or check will be issued by check. Please allow up to 7 business days for check processing. Refunds for payments made by credit or debit card will be returned to the original card used for payment, less any transaction fees, within the timeframe determined by your card issuer.

Changes to These Policies

Fresh Fit Consulting reserves the right to update or modify these Practice Policies at any time. The effective date at the top of this document reflects the date of the most recent revision. Patients will be notified of material changes at their next scheduled appointment or by email. Continued engagement with Fresh Fit Consulting following any update constitutes acceptance of the revised policies.
