When you complete any patient intake form or screening questionnaire hosted through our third party form provider, you may provide personal and health-related information including your name, date of birth, medical history, current medications, symptoms, and insurance information. This information is reviewed by Dr. Boesler and authorized practice staff as part of the intake process and is used to determine the appropriate clinical starting point for your care.
Information collected automatically
When you visit ffcmed.com, certain information is collected automatically through Squarespace's native analytics platform and Google Analytics. This may include your IP address, browser type, device type, pages visited, time spent on pages, and referring website. This information is used in aggregate to understand how visitors interact with our site and to improve its content and structure. It is not used to identify individual visitors.

Health Information and HIPAA
Fresh Fit Consulting is a HIPAA-covered entity. Health information collected through patient intake forms and screening questionnaires is handled in accordance with the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations.
Patient intake forms and screening questionnaires are hosted through a HIPAA-compliant third party platform operating under a Business Associate Agreement with Fresh Fit Consulting. Form responses are transmitted and stored securely and are accessible only to Dr. Boesler and authorized practice staff.
Completed intake and questionnaire responses are retained for a period of 24 months from the date of submission. If you become an active patient of Fresh Fit Consulting, your health information is incorporated into your medical record and retained in accordance with applicable New Jersey medical record retention requirements. If you do not proceed as a patient, your data is deleted at the end of the 24-month retention period.
Contact form submissions are retained for a period of 12 months from the date of submission and deleted thereafter unless they have been incorporated into an active patient relationship.
Email communication through info@ffcmed.com is hosted through Google Workspace under a HIPAA Business Associate Agreement accepted on June 8, 2026. While reasonable safeguards are in place, please be aware that email as a communication medium carries inherent security limitations. Patients with sensitive health-related inquiries are encouraged to use the appropriate patient intake form on our website as the preferred channel for submitting detailed health information.
