FRESH FiT CONSULTiNG
Built on decades of experience in internal medicine and sleep medicine, and tailored for patients who have been through the system and still don't have answers.
Fresh Fit Consulting offers structured testing and physician-guided treatment protocols built around patient outcomes, not insurance guidelines. We know when to look beyond conventional medicine, and we know when conventional medicine is the answer.
FFC operates on a self-pay basis with proprietary in-house protocols. We are not currently accepting new Medicare-based primary care patients.
Little Silver, NJ Private Medical Practice by: iza Boesler M.D.
What is restorative medicine?
Restorative medicine steps outside the scope of conventional insurance guidelines; examining microbiomes, hormonal patterns, inflammatory markers, and toxin exposure to find what conventional testing is not designed to look for. If you've been told your labs are "normal" but you don't feel normal, this is where the real work begins.
Common Patient Concerns at Fresh Fit Consulting
-
The hormonal transition of perimenopause often begins years before a formal menopause diagnosis, disrupting sleep, mood, weight, cognition, and energy in ways that are frequently misattributed or left unaddressed in routine care. FFC offers a thorough hormonal evaluation and physician-guided treatment including bioidentical hormone therapy, individualized to each patient's labs, symptoms, and health history rather than a standardized protocol. Women who are ready to move forward with evaluation and treatment are well suited to our Direct Health Program. Patients who want to understand the full hormonal and metabolic picture before committing to a treatment direction may prefer to begin with the Foundational Health Evaluation.
Relevant testing: Estradiol, progesterone, FSH, LH, DHEA, testosterone, cortisol, thyroid panel

Low testosterone is one of the most common and underaddressed drivers of fatigue, weight gain, reduced motivation, and diminished physical performance in men. Standard panels frequently measure total testosterone only, missing the broader hormonal picture that determines how a patient actually feels day to day. FFC evaluates free testosterone, SHBG, estradiol, and adrenal markers alongside a full clinical assessment to determine whether hormone optimization is appropriate and what form it should take. Patients who already suspect low testosterone and want to move quickly are good candidates for our Direct Health Program. Patients who would prefer a comprehensive whole-body baseline before pursuing treatment may find the Foundational Health Evaluation a more appropriate starting point.
Relevant testing: Total and free testosterone, SHBG, estradiol, LH, FSH, cortisol, thyroid panel

Poor sleep affects hormone regulation, metabolic function, immune response, and cognitive performance over time. As a board-certified sleep medicine physician, Dr. Boesler brings clinical depth to sleep evaluation that goes well beyond what is available in standard primary care. FFC assesses sleep architecture, hormone disruption, adrenal function, and airway physiology to identify what is actually interrupting restorative sleep and builds a treatment plan around the findings. Patients with an existing sleep study seeking more thorough clinical interpretation may be candidates for the Direct Health Program. Those without a prior evaluation are best served by the Two-Step Assessment.
Relevant testing: Home sleep study, cortisol rhythm, thyroid panel, sex hormones, melatonin

Sustainable weight management requires understanding the metabolic, hormonal, and inflammatory factors driving weight gain before selecting a treatment path. FFC develops a physician-guided plan that may include GLP-1 medications such as semaglutide or tirzepatide where clinically appropriate, or structured metabolic alternatives for patients who prefer a different approach or are not candidates for GLP-1 therapy. This is one of the conditions most suited to our Direct Health Program, designed for patients who are ready to move from screening to treatment without delay. Patients earlier in the process who want a full metabolic baseline before deciding on a treatment path may prefer to start with the Two-Step Assessment.
Relevant testing: Fasting insulin, HbA1c, lipid fractionation, thyroid panel, sex hormones, body composition, inflammatory markers

Chronic stress places sustained demand on the adrenal glands, disrupting cortisol rhythm, depleting key hormones, and producing symptoms that range from fatigue and insomnia to anxiety, weight changes, and immune dysfunction. These presentations are difficult to capture through a standard office visit and basic metabolic panel, which is why they are frequently overlooked in routine care. FFC evaluates the full cortisol curve alongside DHEA, sex hormones, and inflammatory markers to build a clear picture of adrenal health and an appropriate treatment response. Patients who want a comprehensive baseline before pursuing treatment often find the Foundational Health Evaluation the most logical starting point.
Relevant testing: Four-point salivary cortisol, DHEA-S, sex hormones, thyroid panel, inflammatory markers

Thyroid dysfunction affects metabolism, energy, weight, mood, and cognition, and exists on a spectrum that standard testing is not always equipped to capture in full. Diagnosis typically relies on TSH as a single reference point, which reflects pituitary signaling rather than the actual activity of thyroid hormones at the tissue level. FFC runs a comprehensive thyroid panel to evaluate the complete picture including active hormone conversion and autoimmune activity. Patients with a prior thyroid diagnosis who feel their condition is inadequately managed are strong candidates for the Direct Health Program. Those presenting with symptoms but no prior diagnosis are best served by the Two-Step Assessment.
Relevant testing: TSH, Free T3, Free T4, Reverse T3, TPO antibodies, thyroglobulin antibodies

Mold toxicity is a well-documented but frequently missed cause of chronic multi-system illness, producing symptoms that include persistent fatigue, brain fog, respiratory irritation, joint pain, and immune dysregulation. The clinical picture often mimics other conditions, contributing to delayed or inaccurate diagnosis. Testing requires specialty panels that fall outside the scope of routine care, and treatment requires a structured physician-guided approach rather than symptomatic management alone. FFC is one of a small number of practices in Monmouth County equipped to evaluate and manage mold-related illness. Given the complexity of presentation, the Two-Step Assessment is the appropriate starting point.
Relevant testing: Mycotoxin urine panel, organic acids, HLA-DR genetic susceptibility, inflammatory markers, immune function

Autoimmune conditions involve the immune system generating a response against the body's own tissue, and managing them well requires understanding both the diagnosis and the factors that may be sustaining or amplifying immune activity. Environmental exposures, gut permeability, hormonal imbalances, and chronic infections are among the contributors that a thorough functional evaluation can identify and address. FFC works with patients who carry an existing autoimmune diagnosis and feel their condition is not adequately managed, as well as patients with unexplained inflammatory symptoms who have not yet received a formal diagnosis. Given the complexity of autoimmune presentations, the Two-Step Assessment is the right starting point.
Relevant testing: Comprehensive inflammatory panel, gut permeability markers, organic acids, environmental toxin screen, hormone panel
