2023 Horizon and Cigna Insurance Coverage Changes

Dr. Iza Boesler is no longer a participating provider within the Horizon & Cigna Networks
Existing and new patients insured by Horizon / Cigna will have the opportunity to see Dr. Boesler on an out of network basis. For all out of network patients, payment is expected at the time of service. A health insurance claim form (CMS 1500 form) will be provided for patients to self submit insurance eligible services. Please see the “Services” page on our website for the most up to date information on service fees and pricing.
Dr. Boesler is a participating provider with Medicare and Tricare. Fresh Fit Consulting will continue to submit claims on behalf of patients with Medicare and Medicare Advantage (PPO) insurance plans.
Please speak to a staff member directly with any questions about your specific circumstances. Patients who are electing not to continue seeing Dr. Boesler are asked to notify the office by 04/30/2023, at which point they will be provided 30 day prescription services while they search for a new in network provider.

Why are you leaving the Horizon & Cigna Health Networks?
The decision to step outside of network coverage was not made lightly. We spent two years attempting to work with Horizon and Cigna who have been categorically non-communicative. While organizations like Horizon and Cigna do increase accessibility of routine medical care, we feel that the structure they impose impedes our ability to best care for our patients. We believe that patients should have access to a variety of treatment options and the final decision in their utilization.
Moving forward, we really see this as an opportunity to focus on expanding patient care instead of an exclusion or a downsizing. We understand that this decision may not be feasible for all of our patients, but hope those who can, continue to elect to trust Fresh Fit Consulting with their health.
